This study looked at how trained community health workers (CHWs) in rural India used digital tools to screen people at home for high blood pressure, diabetes, and anemia. Using point-of-care devices and telehealth support, CHWs screened over 143,000 people across nine states. Many were newly diagnosed; 67.5% with hypertension, 50.4% with diabetes, and 76.2% with anemia. Most participants supported the approach, though some raised concerns about device accuracy and follow-up care. The results show that home-based screening through CHWs, backed by telehealth services, is a practical and scalable way to improve early detection and care for chronic diseases in underserved areas.
