This paper sought to employ a policy capacity framework to better assess the policies underlying the Accredited Social Health Activists (ASHAs) program implemented across rural and urban marginalized communities across India, with a focus on maternal and child care. This CHW program has become the largest in the world, and has focused on enhancing community engagement, strengthening the primary healthcare system, and promoting universal healthcare coverage. Results of this analysis reveal deficits in some of policies which govern the ASHA program, while showing how community/social actors and frontline implementers/CHWs are affected by varying governance arrangements. Authors call for these findings to be considered when fostering the policy capacity of ASHAs and strengthening leadership in India’s healthcare system.
