Amid the global HIV/AIDS burden, community health workers (CHWs) play a pivotal role in high-income countries (HICs), particularly the United States. This scoping review examines CHWs’ impact on HIV outcomes in HICs and identifies evidence gaps in community-based HIV/PrEP services. Out of 1631 abstracts, 34 articles were selected, revealing CHWs’ diverse roles in HIV care, including testing, treatment initiation, adherence, retention, and PrEP services. CHWs employ culturally tailored, community-based, and technology-driven approaches, primarily benefiting Black and Latino populations. Their interventions help bridge healthcare gaps, tackle disparities, and enhance overall health outcomes in HICs.
