This study tested whether using community health workers (CHWs) to link primary care, homes, and schools could improve asthma control for low-income Black children in Philadelphia with asthma. Children received asthma care and support in all three settings over a year. Although asthma improved in all groups, only children who had combined clinic-school support before the COVID-19 pandemic showed a significant improvement in asthma control. This suggests that linking care across all aspects of a child’s life, guided by CHWs, may help manage asthma better in communities facing health disparities.
