This study examined the role of Community Health Workers (CHWs) in helping Medicaid members in underserved Pittsburgh neighborhoods access healthcare and address social needs like food, childcare, and housing. CHWs connected members to doctors, medications, and managed care services. They played a vital role in improving care coordination and addressing barriers to health services. The findings highlight how CHWs can bridge gaps between healthcare systems and communities, making care more accessible and effective. Future research should explore the impact of these programs on patient experience, health outcomes, and costs.
