This review looked at how community health workers (CHWs) and digital tools like phone support are used to help seriously ill adults get palliative care at home, especially in low-resource areas. Out of over 7,500 studies, 31 were included, mostly from the U.S., with few focused on rural or minority groups. CHWs and digital tools were mainly used to support physical and emotional health, especially for people with advanced cancer and other serious illnesses. These services improved quality of life, health care use, and support for caregivers. However, more research is needed to explore culturally tailored care and expand services to underserved communities and countries.
