Community Health Worker Perspectives on Home-Based Care and Remote Monitoring in Heart Failure

This study explored how community health workers help people with heart failure manage their condition at home. It found that patients often struggle with medications, diet, and using health monitoring devices, but community health workers help by explaining care plans, offering emotional support, connecting people to social services, and teaching them how to use technology. The study shows that community health workers can play an important role in improving home care and helping patients stay engaged in their treatment.
