In July 2020, a family medicine clinic developed a system to help COVID-19 patients isolate safely by connecting them to community resources through Community Health Workers (CHWs). This study examined how the system was designed, implemented, and maintained. Interviews with clinic staff showed that flexibility and CHW expertise were key to the system’s success. The main challenge was the lack of coordinated COVID-19 workflows across the healthcare system. Overall, integrating CHWs into the process improved care for patients and the clinic. The study highlights the importance of patient-centered, adaptable workflows for success.
