This trial is testing the impact of Community Health Navigators (CHNs) in helping patients with chronic conditions transition from hospital to home. CHNs, who are trusted local community members, aim to improve care and reduce hospital readmissions by providing support and connecting patients with necessary services. The study involves randomly assigning patients to receive either the CHN support or standard care after discharge, and then comparing outcomes like hospital readmissions and quality of life. Initial feedback from health staff and co-design participants has led to a detailed training program for CHNs. The trial, currently underway in Sydney, Australia, focuses on evaluating the effectiveness and cost of the CHN intervention.
