Advance care planning (ACP) has faced criticism for not significantly improving end-of-life care quality, decision-making, or experiences for patients, families, and clinicians. Despite limited results, stakeholders persist with ACP due to its ethical importance in giving patients a voice in their care. With growing awareness of systemic racism and healthcare inequities, this obligation is increasingly urgent. The challenge remains: how can we make ACP more effective? This study examines the role of Community Health Workers in enhancing effective ACP.
