Community-Based Models for Type 2 Diabetes Care A Review of Effectiveness, Implementation, and Health System Integration

by

Authors: Vandana Esht, Madhur Verma, Shazia Malik, Marim Ali M Slimani, Gunjeet Kaur, Jaya Prasad Tripathy, Gursimer Jeet, Sanjay Kalra

Community-based programs for type 2 diabetes, led by community health workers, peer educators, or digital tools, can help people manage their disease, especially in low- and middle-income countries. These programs improve blood sugar control, weight, and confidence in self-care. Success is higher when programs are integrated into local health systems, culturally relevant, and supported with supervision. Digital tools can reach more people but need ongoing support to stay effective. Scaling up should focus on integration, funding, and training for community health workers.
