Hispanic populations in the US are historically at a higher risk of cardiovascular disease and type II diabetes than their non-hispanic counterparts. This study presents the results of a community-based intervention, Vida Sana Program (VSP), to engage patients and community members in the prevention and treatment of metabolic syndrome by encouraging healthier lifestyles and incorporating the work of navegantes (CHWs). This study highlights the community leaders and navegantes role in the success of this program
