This toolkit highlights the important role of Community Health Workers (CHWs) in supporting people without stable housing. Despite years of policy efforts, homelessness remains a major challenge in the U.S., with unhoused individuals facing serious health issues and barriers to basic needs like housing, food, and safety. Many CHWs have personal experience with homelessness, helping them build trust and connect people to the care and support they need. While CHWs are known as trusted messengers, their work with homeless populations is still not widely recognized or understood. This toolkit shines a light on their efforts to bridge gaps in care and promote health and dignity for people experiencing homelessness.
