Black Family and Community Health Worker Dynamics in a Family-Centric Diabetes Prevention and self-Management Education and Support Intervention

This study found that community health workers helped Black families better manage and prevent diabetes by providing ongoing support over 20 weeks. Families valued the community health workers’ empathy, clear communication, and practical help with understanding health information, using technology, and staying on track with the program. The strong relationships between families and community health workers improved participation and showed that family-based programs with community health worker support can strengthen diabetes care.
