This review examined existing literature to understand the barriers of utilization of health extension workers (HEW) services in Ethiopia and to identify potentially effective interventions to increase service utilization for children ages 0-5 yrs. Papers from 2010 to 2021 were screened with the inclusion/exclusion criteria and categorized by the following themes: (1) levels of utilization or care seeking, (2) barriers and facilitators of utilization and (3) interventions to improve utilization. This review determined a number of demand-side barriers and supply-side barriers (providers) preventing utilization of these services, and suggested strengthening the HEW structure to address these barriers.
