This study examines the integration of Community Health Workers (CHWs) to address health-related social needs (HRSNs) among primary care patients in Bronx County, New York, over 12 months. Using the RE-AIM framework, the study evaluates the reach, effectiveness, adoption, implementation, and maintenance of the CHW intervention. Results show successful outreach to patients reporting HRSNs, with a high proportion receiving social services. Implementation factors, including clinician champion involvement and electronic referrals, are discussed for further investigation.
