Addressing Health-Related Social Needs During COVID-19 Through a Hospital-Based, Community Health Worker Program: A Case Study

A pilot program in Austin, Texas, used Community Health Workers (CHWs) to support Latinx patients hospitalized with COVID-19. Many of these patients had never seen a doctor before and had unmet health needs. CHWs helped by assessing social needs, coordinating care, and providing follow-up support. Patients, most of whom spoke Spanish, were highly satisfied with the program and reported fewer unmet needs. Healthcare providers praised CHWs for their role in connecting patients to resources and improving care. The study highlights the need for hospital-based CHW programs to reduce health disparities in vulnerable communities.
