This study explored how a New York City program, the Health Justice Network (NYC HJN), connects people with justice involvement to health and social services through community health workers (CHWs). Interviews with CHWs and staff revealed that CHWs’ lived experience, the program’s flexible requirements, and its focus on participants’ needs helped make the program successful. However, challenges included the lack of a formal implementation guide and varying degrees of support and integration for CHWs at partner sites. These insights can guide future CHW-led programs to better serve people reentering society.
