This project utilized a human-centered design to implement a community health worker (CHW) intervention in permanent supportive housing (PSH) for formerly chronically homeless adults. The aim was to reduce food insecurity and improve nutritional outcomes. Food-security screening identified 64% of residents with low food security. Major themes from resident feedback included kitchenware/appliance needs, knowledge gaps, positive perceptions of healthy options, and community re-entry. The pilot project successfully engaged residents in adopting healthy practices and fostering social connectedness, emphasizing the need for further research in addressing food insecurity among homeless populations in supportive housing.
